Runway Model Application & Parental Consent
Dear Parent/Guardian,
We’re excited to introduce the first-ever Bentonville Fashion Week, an event celebrating fashion, creativity, and emerging talent. This event is being organized in partnership with a nonprofit organization that is fundraising for Mental Health Awareness Month in May.
Your child has the opportunity to apply for a spot as a runway model, where they may be selected to walk in designer showcases. Please note that this is a volunteer opportunity, and models will not be paid for participation. However, this event will be a fantastic experience to support a great cause while celebrating fashion in Bentonville.
As part of this application process, we require parental consent and key details about your child’srunway experience and availability. Please note that filling out this form does not guarantee selection, but it ensures they are considered for the show.
By completing this form, you:
✔️ Allow your child to be considered as a runway model for Bentonville Fashion Week.
✔️ Provide essential details to help us match models to designer collections.
✔️ Confirm their availability for fittings, rehearsals, and shows if selected.
✔️ Approve or decline media usage for promotional purposes.
If selected, you will receive an email with further details on model assignments, fitting schedules, and rehearsal dates.
Audition Details:
Auditions will take place on March 16th from 3:00 PM - 6:00 PM at the Embassy Suites Hotel – Razorback Conference Room located at:
3303 Pinnacle Hills Pkwy, Rogers, Arkansas 72858.

NOTE: If a Model is under 18 years of age. This form must be printed, signed, brought to
BFW 2025 Model Casting Call, and turned in to our staff.
